The HSC Foundation


The HSC Foundation is dedicated to improving access to services for individuals who face social and health care barriers due to disability, chronic illness, or other circumstances that present unique needs. The HSC Health Care System is a multi-faceted organization that weaves together a care coordination plan (Health Services for Children with Special Needs, Inc.), a pediatric specialty hospital (The HSC Pediatric Center), a home health agency (HSC Home Care, LLC) and a consulting company (Special Needs Consulting Services) and the parent organization, The HSC Foundation. Our unique system offers a comprehensive approach to caring, serving, and empowering individuals with disabilities.


National Youth Transitions Initiative
This initiative assists youth and young veterans with disabilities in creating and living a self-directed path to adulthood and employment. The National Youth Transitions Initiative is the signature initiative of The HSC Foundation.

Family Supports
The HSC Foundation is committed to supporting families and other individuals who are providing care for infants, youth, and adults with chronic illness, disabilities, at-risk for disabilities, or who have unique health care needs.

Advocates in Disability Award (ADA)
The ADA Program awards and encourages a young individual with a disability between the ages of 14 and 26, who has dedicated himself/herself to positively affecting the lives of individuals with disabilities and their families in the United States. The Program also supports an innovative project developed by this young person with a disability that serves and empowers individuals with disabilities.
