This is an organization in Kentucky that partners with people with disabilities to plan, create and sustain individualized custom supports.
From the website:
"Realizations provides person-centered supports. We do not have pre-existing programs or services (e.g., residences or day programs) into which we try to fit people. Instead, we spend time getting to know people and helping them determine the kind of life they want to live. Unique supports are then designed based on the specific help people will need to live the life they want to live. As a result, people live in their own homes and spend their days working, learning, contributing, socializing, pursuing interests, getting healthier, managing their household, worshiping, etc."
Realizations also puts on an annual week long event called the "Summer Institute on Community" in Louisville, KY. This event is designed to inform, inspire and give participants the opportunity to interact with like-minded others.
