Required Product Information

The following information is needed to complete the long nomination form:

1. Your name and either a telephone number or e-mail address where you can be reached.
2. The full, accurate name of the product being nominated.
3. A brief description (just a few words) to identify the product in search results and index pages.
4. A product overview of about 100 words that will appear on the product overview page. This should be clear and concise and easy to read. It may be written or revised by QualityMall.org staff.
5. Store(s) and Department(s) where the product will be listed. You MUST indicate a store for your product. If you can not place your product in an appropriate store, it is possible that your product does not meet the inclusion criteria for QualityMall.org. To see a list of stores, with descriptions and representative products, browse the store directory.
6. A full description (500-750 words) with more detailed and in-depth information on the product.
7. Complete contact information for the product. This should be someone directly associated with the product being nominated. If it is a material or resource that can be purchased or ordered, it should be the correct contact for ordering the product.
8. Web link(s), if any, for the Web site featuring or selling the product, the web presence for the organization sponsoring the project, etc. Use only links with a direct connection to the product (and not links to similar products or organizations).
9. Key words that will help users locate the product using a search tool.
10. Whether or not the product is accessible and directly useful to people with disabilities.
11. Media type(s) when appropriate
12. Area(s) served by the product.
